Intra Cytoplasmic Sperm Injection (ICSI)
Intra Cytoplasmic Sperm Injection (ICSI) is a specialized assisted reproductive technique primarily used for male factor infertility. In this procedure, a single sperm is carefully injected directly into the cytoplasm of a mature egg using a highly precise instrument called a micromanipulator. ICSI significantly improves fertilization rates and provides hope to couples facing challenges such as very low sperm count, poor sperm motility, or abnormal sperm morphology. It is also beneficial for women with a limited number of eggs, older women, or couples who have experienced failed fertilization in previous IVF cycles.
The ovarian stimulation and monitoring for ICSI follow the same protocols as a conventional IVF cycle. Eggs are retrieved following controlled ovarian stimulation, and fertilization is performed in the laboratory under highly controlled conditions. The resulting embryos are then monitored for quality and subsequently transferred into the uterus, similar to the standard IVF process.
Surgical Sperm Retrieval Techniques
In cases where sperm is extremely limited or absent in the ejaculate, known as azoospermia, surgical sperm retrieval techniques are employed. These procedures allow viable sperm to be obtained directly from the testes or epididymis for use in ICSI.
Testicular Biopsy is an in-office surgical procedure where small tissue samples are taken from the testicle and examined for viable sperm.
Testicular Sperm Aspiration (TESA) involves inserting a fine needle into the testicle to extract tissue containing sperm, which is then used for fertilization in ICSI.
Percutaneous Sperm Aspiration (PESA) targets the epididymis, where a needle is used to aspirate sperm directly.
Any surplus sperm or testicular tissue retrieved through TESA or PESA can be cryopreserved for future use, ensuring multiple attempts can be made without repeating surgical procedures.
